Part-Time Clinical Assessment RN
Location: In-home assessments in Lowell, Lawrence, Andover, and Tewksbury, MA
Schedule: Monday-Friday, 9:00 AM-5:00 PM (24-30 hours/week; 3-4 days/week)
Pay: $57/hr
Mileage: Reimbursed at $0.70/mile
Duration: 13 weeks to start (Temp-to-Perm & Direct Placement options available)
Position Summary
Amergis Staffing is partnered with a much respected Adult Day Health organization providing community-based care services that help elderly and disabled individuals remain safely and comfortably in their homes. Their programs support both clients and caregivers through customized care plans, clinical oversight, and dedicated care management resources.
The Clinical Assessment RN plays a key role in ensuring client safety and stability in the community by conducting in-home assessments, developing care plans, and collaborating with an interdisciplinary Care Team to reduce unnecessary hospitalizations.
Key Responsibilities
Complete intake, annual assessments, and admission visits.
Complete all documentation including client assessments, multidisciplinary plans of care, and communication with PCPs and related providers.
Review and educate caregivers on the admission binder and required communication tools (paper or EVV, depending on program).
Conduct regular in-home RN visits, caregiver training, and assessments of health or environmental changes.
Perform semi-annual visits: update plans of care, review goals, complete medication review/reconciliation, and audit caregiver communication tools.
Identify and address concerns related to safety, psychosocial well-being, and physical health changes.
Participate in interdisciplinary team meetings and collaborate to support client needs.
Foster a culture of customer service, compassion, and quality care.
Serve as a brand ambassador for Vitra Health, upholding the organization's mission and values.
Demonstrate empathy, cultural awareness, and commitment to diverse populations.
Other duties as assigned.
Qualifications
Active Massachusetts RN license in good standing.
2+ years of RN experience preferred.
Experience conducting clinical assessments required.
Strong clinical judgment and care delivery skills.
Intermediate-advanced proficiency with Microsoft Office Suite.
Willingness to utilize telehealth tools (video, chat, etc.) when appropriate.
Preferred Experience:
Electronic medical record experience strongly preferred.
Background in care management is a plus.
Working Conditions
Work occurs in residential and community settings.
Travel required to member homes; occasional remote work possible.
Valid driver's license; ability to travel throughout Massachusetts.
Member-Facing Role: YES - direct in-person care.
