In this role, you’ll lead essential responsibilities that align with best practices in medical social work. You’ll conduct psychosocial assessments, identify barriers to recovery, coordinate discharge planning, link patients and families to community resources, and advocate for necessary supports to promote timely, safe, and sustainable transitions of care. You’ll collaborate with multidisciplinary teams to align clinical and psychosocial goals, document clearly, and ensure continuity across settings.
